One day, researchers at the Institute of Medicine (IOM) uncovered a startling truth: over 48,000 to 98,000 lives were being lost annually in the U.S. due to medical errors. That’s like a jumbo jet crashing every day! This revelation sent shockwaves through the healthcare world.

A Quest for a Safer System
Instead of pointing fingers, these researchers asked a groundbreaking question: What if we redesigned the system itself? They likened it to making roads safer for even the best drivers. This idea ignited a movement to transform healthcare—not by punishing mistakes, but by preventing them.

A Cultural Revolution in Healthcare
The IOM’s report, “A story of change in Health care (Adapted from the Institute of Medicine report 2000 ), became a rallying cry for change. It championed a cultural shift where healthcare embraced safety, teamwork, and continuous learning.
